Administrative Rescheduling Request

If you would like to request to reschedule your hearing, please download and complete the document above. This request has no effect unless granted by the department. Additionally, in no event may the hearing begin later than 90 days after receipt of the hearing request. Be sure to state the reason(s) you cannot attend your hearing date.

Please submit your request to Neighborhood and Business Safety Division via email.

Hearing By Written Declaration

If you would like to request a hearing by written declaration in lieu of attending a hearing in person, please complete the documentation above. If you choose to participate in writing, please deliver or mail your documentation no later than the time and date of your hearing.
